Fred Hutchinson Cancer Research Center’s Survivorship Program provides clinical care, patient education and research opportunities to patients who have survived cancer and are not in active cancer treatment or are in long-term therapy to prevent recurrence.
Journey Forward is a program designed for doctors and their patients who have recently completed active treatment for cancer. This program was created by a unique collaboration of organizations with the common goal of improving survivorship care.
While survivorship plans are extremely important, the amount of time they would require makes them unachievable for most oncology practices. OncoLink hopes to take steps to making this care plan a reality by developing a basic plan that patients who have recently completed active treatment can review and discuss with their healthcare team.
