The Living Beyond Breast Cancer (“LBBC”) gift card giveaway (the "Sweepstakes") is sponsored by LBBC (owner and operator of www.lbbc.org) and having an address of 40 Monument Rd, Suite 104, Bala Cynwyd, PA 19004 ("Sponsor").
ELIGIBILITY
Open only to permanent legal U.S. residents physically residing in the fifty (50) United States of America and the District of Columbia, who are eighteen (18) years of age or older or the age of majority in their states of residence (which is nineteen (19) in Alabama, Nebraska; twenty-one (21) in Mississippi; and eighteen (18) in all other states) as of the date of entry. Employees, directors, and officers of Sponsor and its parent, affiliates, subsidiaries, advertising and promotion agencies, and any other entity involved in the development or administration of this Sweepstakes, and the immediate family members of each such person (spouse, parent, child, sibling, grandparent, and spouse or "step" of each and their respective spouses, regardless of where they reside), and those living in the same household of each such person (whether related or not), are not eligible to participate in or win a prize. By participating, entrants agree to abide by and be bound by these Official Rules and the decisions of the Sponsor. Sweepstakes may only be entered in or from the fifty (50) United States and the District of Columbia, and entries originating from any other jurisdiction are not eligible for entry.
Winners may be required to sign an Affidavit of Eligibility and complete relevant tax forms as a condition to the delivery of the applicable prize. Void where prohibited by law. All applicable federal, state and local laws apply.
HOW TO ENTER
No purchase or donation is necessary to participate in the Sweepstakes. There are two methods of entry, via online and via mail.
Online entry: To enter, fill in the LBBC online Evaluation Form at www.lbbc.org every time you participate in a LBBC program/activity (such as watch a virtual program, attend a webinar, visit web pages of LBBC.ORG) and submit and validate an email address using the Enter to Win form as provided at www.lbbc.org between 12:00 a.m. EST on January 1, 2022, and 11:59 p.m. EST on December 31, 2022 (the "Entry Period"). Entries generated by script, macro or other automated means are void. Normal time, toll, connection and usage rates, if any, charged by your Internet service provider will apply. Sponsor is not responsible for any incomplete entries as further described below.
Mail entry: Request a paper copy of LBBC’s Evaluation Form every time you participate in a LBBC program/activity by writing to Living Beyond Breast Cancer, 40 Monument Rd, Suite 104, Bala Cynwyd, PA 19004 or by calling (885) 807-6386, complete the LBBC Evaluation Form and the Enter to Win form and submit it by mail during the Entry Period to: LBBC Evaluation Sweepstakes, 40 Monument Rd, Suite 104, Bala Cynwyd, PA 19004. Mail-in entries must be received by the last day of the Entry Period. No photocopies or mechanically reproduced entries allowed. Sponsor is not responsible for entries or mail that is late, lost, misdirected, mutilated, incomplete, delayed, garbled, stolen, inaccurate, illegible, or postage-due or as further described below.
Each entry submitted with a completed Evaluation Form and Enter to Win form in accordance with these Rules will constitute an official entry for the month submitted. Entries not selected to win are not carried over to the next month’s drawing. At least one new Evaluation Form and Enter to Win form must be submitted each month to be eligible to win during the entire Entry Period. There are no limits to the number of entries per person or household which may be submitted each month or in total during the Entry Period.
All entries become the property of the Sponsor and will not be returned. All information collected as part of the Sweepstakes is subject to Sponsor’s Privacy Policy posted on Sponsor’s website and available in paper form upon request.
All registrations must include entrant’s valid email address or postal address. The email address provided by online entrants will be the identity of the entrant and, if selected for a prize, the identity of the winner. The name and postal address provided by mail-in entrants will be the identity of the entrant and, if selected for a prize, the identity of the winner. Sponsor expressly reserves the right to disqualify any entries believed in good faith to be generated by an automated program or via scripts. By participating, all entrants agree to abide by these Official Rules.
PRIZES
One winner per month will be selected during the Entry Period. Each winner will receive one (1) Fifty Dollar ($50) Gift Card to a store or merchant selected by Sponsor. Sponsor may select a different store or merchant for each Gift Card awarded.
Decisions of the Sponsor are final and binding with respect to all matters related to the Sweepstakes. All prizes may not be awarded. In no event shall the Sponsor be obligated to award more prizes than the number of prizes stated in these Official Rules. No substitutions (including for cash) are permitted, except that Sponsor reserves the right to substitute a prize of equal or greater monetary value for any prize.
Winners shall be responsible and liable for all federal, state and local taxes on the value of their prize. To receive a complete list of winners or a copy of the Official Rules, send a self-addressed stamped envelope to:

Specify winner’s list or rules on your request. A complete winner’s list will be available after January 15, 2022. All requests for the winner’s list must be received no later than January 31, 2022. Allow four (4) weeks for prize delivery.
SELECTION AND NOTIFICATION
The winning entries will be chosen by Sponsor at random each month during the Entry Period. Selection of the winner for each month will be from entries received before the last day of such month and will made by the 5th day of the following month. The number of eligible entries received each month via all methods of entry combined determines the odds of winning.
The Sweepstakes will be conducted under the supervision of the Sponsor. The decisions of the Sponsor are final and binding in all matters relating to this Sweepstakes. Winners will be notified by email for online entries and by mail for mail-in entries; however, Sponsor reserves the right to modify the method of notification. Winners must claim their prize within seven (7) business days after the date of notification of such prize. Any notification of prize returned to Sponsor as non-deliverable and/or failure of the winner to respond to the prize notification within the specified seven (7) business days will be considered a forfeiture of the prize and an alternate winner may be selected from the pool of eligible entries or the prize may remain unawarded. If an entrant is found to be ineligible, an alternate winner may be selected from the pool of eligible entries or the prize may remain unawarded. Sponsor reserves the right to ship prizes directly to the mailing address provided by winners during their entry to the Sweepstakes.
GENERAL CONDITIONS, LIMITATIONS OF LIABILITY, AND RELEASES
This Sweepstakes is conducted in English. An entrant or winner may be disqualified from the Sweepstakes if she fails to comply with each provision of these Official Rules, as determined in the sole discretion of the Sponsor. Participation in the Sweepstakes is at entrant’s own risk. Sponsor shall not be liable for: (1) failed, returned or misdirected notifications based on inaccurate information provided by the winner on the Sweepstake entry form; (2) entries and responses to winner notifications which are lost, late, incomplete, illegible, unintelligible, postage-due, misdirected, damaged or otherwise not received by the intended recipient in whole or in part or for computer or technical error of any kind; (3) any electronic miscommunications or failures, technical hardware or software failures of any kind, lost or unavailable network connections, or failed incomplete, garbled or delayed computer transmissions which may limit an entrant’s ability to participate in the Sweepstakes; (4) any technical malfunctions of the telephone network, computer on-line system, computer equipment, software, program malfunctions or other failures, delayed computer transactions or network connections that are human, mechanical or technical in nature, or any combination thereof, including any injury or damage to entrant’s or any other person’s computer related to or resulting from downloading any part of the Evaluation Form or this Sweepstakes; or (5) any warranty of fitness or merchantability of any prize or the function or operation thereof, which shall be the sole responsibility of the store, merchant, or other provider of the prize.
Sponsor does not make any, and hereby disclaims any and all, representations or warranties of any kind regarding any prize. Unless prohibited by applicable law, your entry constitutes your permission for Sponsor to use your name and address (city and state) in connection with the Sweepstakes without any further compensation to such entrant. By entering or accepting a prize in the Sweepstake, winners agree to be bound by these Official Rules and to conform to all federal, state and local laws and regulations. When applicable, the winner may be required to execute and return to Sponsor within ten (10) business days an Affidavit of Eligibility and a Liability and Publicity Release to be eligible for the prize or an alternate winner will be selected. Winner may be required to furnish proof of identity, address, and birth date in order to receive a prize.
By entering, an entrant agrees to release and hold harmless Sponsor and its respective parent, subsidiaries and affiliated entities, directors, officers, employees, attorneys, agents, and representatives from any damage, injury, death, loss, claim, action, demand, or other liability (collectively, "Claims") that may arise from acceptance, possession and/or use of any prize or their participation in this promotion, or from any misuse or malfunction of any prize awarded, regardless of whether such Claims, or knowledge of the facts constituting such Claims, exist at the time of entry or arise at any time thereafter.
Any person attempting to defraud or in any way tamper with this Sweepstakes will be ineligible for prizes and may be prosecuted to the full extent of the law. Sponsor reserves the right to modify these Official Rules in any way or at any time. Sponsor reserves the right, in its sole discretion, to cancel or suspend this Sweepstakes should viruses, bugs or other causes beyond its control corrupt the administration, security or proper function of the Sweepstakes. In the event of cancellation or suspension, Sponsor shall promptly post a notice on the Sweepstakes entry page to such effect. This Sweepstakes shall be governed by Pennsylvania law. By participating in this Sweepstakes, entrants agree that Pennsylvania courts shall have jurisdiction over any dispute or litigation arising from or relating to this Sweepstakes and that venue shall be only in Pennsylvania.
